I also hereby voluntarily consent to SBF publishing my personal data to the extent of: first name, surname and registration number, for the purpose of publicly verifying my identity for the duration of my SBF membership. I further consent to SBF processing, including publishing, my personal data to the extent and for the purposes set out in Act No. 440/2015 Coll. on Sport, in particular Section 79 et seq. SBF may provide my personal data to the Ministry of Education, Science, Research and Sport of the Slovak Republic (hereinafter the “Ministry”) under Section 79 et seq. of Act No. 440/2015 Coll. on Sport. SBF undertakes to process personal data in accordance with Act No. 122/2013 Coll. on Personal Data Protection. I am aware of my rights under Act No. 122/2013 Coll. on Personal Data Protection, of which I have been informed. I am aware that I may withdraw my consent in writing at any time, in particular if SBF or the Ministry breaches its obligations concerning the protection and security of personal data under Act No. 122/2013 Coll. on Personal Data Protection. However, withdrawal of consent may lead to the cancellation of my registration with SBF.
